8/19/2023 EnchantaVaughn Approved Behavior: Upon arrival, client appeared to be in an ecstatic mood, as evidenced by his enthusiasm and positive expressions. Client was seen displaying appropriate social interactions in the session. Client maintains an excellent attitude, demonstrating a good rapport with QMHP. Client engages well, communicating positive affirmations in session. Client exhibits great participation, following rules and expectations the first time. Client was supported for increasing his involvement in social and recreational opportunities. Intervention: QMHP inquired about client’s day and what he is doing to make it a good day. QMHP monitored client individualized behavior modifications in collaboration with his siblings and guardian. QMHP assisted client with finding and utilizing necessary resources in his personal environment. QMHP praised client for staying on task. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of his home sessions and how it is supposed to guide him into his daily life habits and choices. QMHP continued to exxpress to client on how important it is for him to be able to communicate with his mother his feelings as well as implement what he is being taught on a daily basis. Response: Client exhibited a positive attitude toward mother and counselor. Client smiled and hugged QMHP for being there for him when he needed. Client displayed positive interaction with his siblings and was supported with building healthy relationships with his family. Client presents a positive attitude when given the opportunity to earn a reward. Client implemented the positive reinforcement provided by his mother and was able to stay motivated and enjoy his session.
